Community Living Center

The Community Living Center (CLC) operates three units (88 beds) on the first floor of the medical center. Transitional care is provided and includes skilled nursing care, medical management, pharmaceutical and laboratory service, intravenous therapies, radiation therapy, rehabilitation services, end-of-life care, respite care, and geriatric evaluation and management.

CLC has an average length of stay of 29–30 days and operates under the guidance of TJC Long Term Care Standards. Each patient has an individualized plan of care developed by an interdisciplinary geriatric team including primary care providers and representatives as appropriate from nursing, nutrition and food service, occupational, physical, speech, and recreational therapy, dental, social work, pharmacy and chaplain service. The majority of patients are discharged to their home environment.

Typical diagnoses or conditions treated:

Complex medical problems requiring rehabilitation and medical management (e.g. emphysema, renal failure, diabetes mellitus, congestive heart failure, malignancy requiring radiation therapy or chemotherapy, medical conditions with depression co-morbidity); wound care (e.g. post-surgical, decubitii, diabetic foot ulcers, and peripheral vascular disease); and recent orthopedic surgery requiring rehabilitation services.

Types of services most frequently provided:

Skilled nursing care, primary medical care by physicians and mid-level practitioners, rehabilitation therapies (PT, OT, Speech, and Recreation), complex medical management, radiation therapy, wound care, prosthetic training, management of geropsychiatric conditions, comprehensive general geriatric evaluation and management, discharge planning, nutritional management, and medication review.

Hours of operation: CLC provides direct patient care services 24 hours a day, seven days a week.
